Practice Information

The staff at Creekside will assist individuals, families, professionals, and agencies in coordinating referrals for evaluation. The office is open Monday through Friday, 9:00 AM to 5:00 PM. Our answering system is on from 12:00 PM to 2:00 PM, evenings after 5:00 PM, and weekends. Phone messages will be returned as soon as possible.

Dr. Knowles is a participating Blue Cross/Blue Shield provider.  

Creekside professionals are committed to providing excellent mental health care to our patients. The recent changes in the health insurance system may affect our decision to participate with different insurance providers; however, it will not impact the quality of care that we will continue to provide to our patients. If you are a beneficiary of Blue Cross Blue Shield of Florida, please review the following notice of the termination of their agreement with our clinicians.
