Everyone working at Creekside understands their responsibility to maintain the confidentiality of your treatment. In general, when a request for information is received from an outside party, we release only the minimum information necessary. Response to official requests from courts and attorneys regarding your treatment is required by law. Insurance carriers request information about your treatment to help ensure quality care. We provide that information with your permission; declining to release information may affect your reimbursement. Patients frequently request letters or other reports to inform schools, employers, or insurance reviewers about their treatment. No records are ever released without your permission.
We also welcome input from family members, teachers, and others involved in a patient's care, as this information is often helpful for evaluation and treatment. Any information you'd like us to consider can be shared by phone, through our voicemail system, or in writing while your own records remain confidential and are never released without your permission.
