Fees

Charges for professional services are based on time spent and the nature of the service provided. Your clinician will discuss fees with you, and you are encouraged to discuss your concerns with us.  Fees for consultation with attorneys, teachers, and other agencies will be discussed before the service is provided. Any telephone consultation fees added to your account should be paid at your next visit.

Frequently, patients request letters or other reports to better inform schools, employers, or insurance reviewers about their treatment.  No records are ever released without your permission.  Creekside has an administrative fee of $25.00 for each request for information.  Some requests may incur additional proffessional fees.

Payment can be made by cash, check, or major credit card and is expected at the time of service unless prior arrangements have been made.

Insurance

Patients and families are responsible for professional fees. Insurance companies have a bewildering array of deductibles, co-pays, limits on the number and frequency of visits. We will help confirm benefits with your insurance carrier but we do not rely on the accuracy of information provided by your insurance company.


Please bring all insurance cards with you to your first visit. Payment of deductibles and co-payments is expected at the time of each visit. We will file your insurance and send monthly Creekside account statements.

Many insurance plans require patients or family to obtain authorization for mental health treatment. 

Creekside does call the new patient’s insurance company prior to the first appointment, as a courtesy, to obtain benefits information. Please note that the benefits quoted to us by the insurance company are not a guarantee of payment. Payment by the insurance company is subject to the terms and conditions provided within the contract between the patient and the insurance company.
