A: Patients and families are responsible for professional fees. Insurance companies have a bewildering array of deductibles, co-pays, limits on the number and frequency of visits. We will help confirm benefits with your insurance carrier but we do not rely on the accuracy of information provided by your insurance company. Please bring all insurance cards with you to your first visit. Payment of deductibles and co-payments is expected at the time of each visit. We will file your insurance.
A: We will be happy to communicate with your doctor or other therapists to facilitate quality mental health treatment. The type and level of the communication will vary depending on the specific circumstances of each patient. We will work with you and your other health providers to define these levels to suit your needs.
A: Family support is integral to the success of mental health treatment. Information from parents, teachers, guardians, caregivers, and others involved in the care of children and some adults is necessary for adequate evaluations and treatment. Some family members may be included in the evaluation and treatment plan, at other times sending notes or leaving a detailed message in our confidential voice mail system may be convenient. Communication with family and others is essential when patient safety requires additional structure, when maltreatment of a child is suspected, or in other situations required by law. At the same time, all patients have the right to a confidential professional relationship. We will help define the levels of communication that will be important to your treatment.
A: Records keeping for prescription refills that are not associated with office visits has become an increasing burden. Some refill requests are directly from patients. Others come from a pharmacy or mail-in service. There is an administrative charge of $20.00 for any request for a prescription refill, regardless of the source of the request. It will be important for you to keep track of the number of refills written, schedule your follow-up appointments in a timely manner, and consider the use of mail-in services for medications taken for more than three months. If you are needing a prescription refill, please call our prescription refill line at 850.476.1392. PLEASE NOTE: Prescriptions are not refilled after hours or on weekends. Prescription refills are processed during office hours in order to maintain accurate records.
A: The Clinical Services Coordinator and support staff are available between 9:00 a.m. and 5:00 p.m. Monday through Friday at 850.476.0977. Messages left after 5:00 p.m. or on weekends will be returned on the next working day. Call 850.476.0977 for an urgent problem after hours, a clinician from Creekside is available to help you. The Pensacola office phone is answered 24 hours a day with information for obtaining immediate assistance. PLEASE NOTE: Prescriptions are not refilled after hours or on weekends.
A: Most patients and families keep scheduled appointments which supports the effectiveness of treatment. At times appointments are cancelled with little notice to our office, or not kept. Please contact our office as soon as possible to inform us if you will be unable to make an appointment. Creekside will charge an administrative fee of at least $25 for late cancellations and no-shows. Patients might also be charged the full cost of the office visit in addition to the administrative fee. We will make every effort to accommodate your schedule. We do provide phone reminder 48 hours before the appointment date. We can provide other reminders for your appointment, if necessary.
A: Frequently, patients request letters or other reports to better inform schools, employers, or insurance reviewers about their treatment. No records are ever released withour your permission. Creekside has an administrative fee of $25.00 for each request for information. Some requests may incur additional professional fees.
A: Each of our clinicians have years of education and experience in the fields of psychiatry, psychology, social work, and counseling. We have two psychiatrists and four psychiatric social work therapists available to provide state of the art outpatient mental health care.
