Appointments for new patients are set by the clinician. Basic clinical information is requested at your first call. Please arrange for any previous treatment records to be sent to us. If we are not able to provide treatment at Creekside our Clinical Services Coordinator can help you with a referral. Please go to the Referrals section of this site if you are interested in seeing one of the clinicians at at Creekside.
Follow-up appointments are scheduled with the receptionists. We will work to accommodate your other commitments. Please arrive 15 minutes prior to your initial visit to complete registration forms.
Every effort is made to begin and end sessions on time. The office should be notified promptly of any changes in appointments. We require 24 hours notice to cancel appointments. There is an administrative charge for missed appointments or those cancelled with less than 24 hours notice.
We will make every effort to accomodate your schedule. We do provide a reminder 48 hours before the appointment date. We can provide other reminders for your appointment, if necessary.
