Planning for Your Visit

Appointments for new patients are set by the clinician. Basic clinical information is requested at your first call. Please arrange for any previous treatment records to be sent to us. If we are not able to provide treatment at Creekside our Clinical Services Coordinator can help you with a referral.  

Our professionals will review information about your current treatment needs and consider how best to help you. We will respond to your request for help, but our review of this information does not establish a treatment relationship with any provider at Creekside. It is possible that your problems would be better addressed in a different setting.

Every effort is made to begin and end sessions on time. The office should be notified promptly of any changes in appointments. We require 24 hours notice to cancel appointments. There is an administrative charge for missed appointments or those cancelled with less than 24 hours notice.

Forms for your visit
We use these forms to gather information we need to begin the evaluation process. There is a set for adults and another for children. In general, adults should provide this information if they are seeking treatment. Parents may request treatment for their children and teenagers. We may require permission from older adolescents in order to talk with their parents when considering a new referral.
 You can use this Link to electronic versions of these forms, or you may print out the forms then send them by mail or by fax to (850) 476-2558. There is an Authorization for Release of Information included in this packet so that you can begin the process of obtaining information from any previous or current mental health treatment providers.
