The staff at Creekside will assist individuals, families, professionals, and agencies in coordinating referrals for evaluation. Our professionals will review information about your current treatment needs and consider how best to help you. We will respond to your request for help but our review of this information does not establish a treatment relationship with any provider at Creekside. It is possible that your problems would be better addressed in a different setting.
In general, adults should provide this information if they are seeking treatment. Parents may request treatment for their children and teenagers. We may require permission from older adolescents in order to talk with their parents when considering a new referral.
We use forms to gather the information we need to begin this process. There is a set for adults and another for children. These forms require Adobe Reader which you can download by clicking on the following link.
You may print out the forms and mail them, or send them to our secure FAX (850) 476-2558. There is an Authorization for Release of Information included in this packet so that you can begin the process of obtaining information from any previous or current mental health treatment providers.
